We’re a small medical practice — Marin Family Medicine Medical Group, Inc. (“Marin Family Medicine,” “we,” or “us”) — and we treat information from our website visitors the way we’d want ours treated. This page explains, plainly, what we collect on this website and what we do with it.
This policy describes what we collect through the marinfamilymed.com website and how we use it.
It does not cover your medical records or the care you receive as a patient. Health information you share with us as a patient — through visits, the patient portal, or secure messaging — is protected health information under HIPAA and is governed by our Notice of Privacy Practices, which we provide when you join the practice.
When you fill out our interest form, we collect your name, email address, phone number, and anything you write in the questions field. We use it to respond to you, tell you about membership, and occasionally share practice updates by email. You can unsubscribe from updates at any time.
When you schedule an appointment or an introductory call, we ask a few questions — including what you’d like to talk about — so we can prepare for your visit. Scheduling is processed by Acuity Scheduling under a Business Associate Agreement, which requires them to protect your information the way we do, and everything you enter is stored securely.
If something feels too sensitive for a form, you’re welcome to leave it out and tell us by phone instead. Current patients can always message their care team directly — that channel is secure and part of your medical record.
Like most websites, we use Google Analytics to understand how visitors use the site — which pages are viewed, roughly where visitors come from, and what device they use. Google Analytics uses cookies; the reports we see are aggregated and don’t identify you personally. Our hosting provider also keeps standard server logs (such as IP addresses) for security and reliability.
You can limit cookies through your browser settings. We also honor “Do Not Track” and Global Privacy Control browser signals — if your browser sends one, we turn analytics off entirely for your visit.
We do not sell, rent, or trade your personal information. We do not run third-party advertising on this website, and we don’t share your information with anyone for their marketing.
We share information only with the service providers that make the website and our communications work: our website host, Google Analytics, Acuity Scheduling, our secure messaging provider, and our email service. Each receives only what it needs to do its job. We may also disclose information if the law requires it.
You can ask us at any time what information we hold about you from this website, ask us to correct it, or ask us to delete it — email admin@marinfamilymed.com and we’ll take care of it. Every marketing email we send includes an unsubscribe link.
Our website is intended for adults. We don’t knowingly collect personal information from children through the website — parents and guardians enroll children in the practice.
We use reasonable safeguards to protect the information collected through this website, including encrypted connections (HTTPS). No method of transmission over the internet is completely secure, so we can’t guarantee absolute security — another reason to keep detailed health information to secure patient channels.
